Description

Objective:

The course objective is to conduct analysis of various aspects of mishap investigations such as recent theories associated with mishap causes, investigative techniques, data acquisition, and structure of investigative reports, management responsibilities, and remedial actions. Particular emphasis is placed on determining the sequence of events to develop management actions which will prevent recurrence of mishaps.

Learning Outcomes:

  • Define the major aspects of site management at a mishap scene
  • Confidently interact with the media at the mishap site and during the on-going investigation
  • Coordinate with local area responders
  • Conduct site documentation and management including, analysis, photo documentation, cognitive survivor interviews and witness reports
  • Distinguish the difference between structural failures that may have caused the mishap and structural damage that occurred as a result of the mishap
  • Analyze fire, explosions, weather, Radar, survival factors and human performance
  • Develop findings and recommendations using the Modern Accident Causation model, and the Department of Defense Human Factors model
  • Discuss the entire accident investigation process, from when the department head receives the initial notification that a mishap has occurred through the issuance of the final report and determination of the mishap's findings and recommendations
  • Conduct a formal mishap investigation board
  • Perform effective witness questioning techniques to elicit the most accurate information
