Description

Objective:

In the current healthcare system model the role of a Care Coordinator as an essential care provider has grown beyond just the management of chronic conditions. Today Care Coordination focuses on accessible and consistent access to care, while empowering patients to take control of their own wellbeing. This course will teach Care Coordinators to respond to the diverse needs of the patient populations, as well as provide an overview of the essential roles and responsibilities of a Care Coordinator.
