Acceptable Documentation for Awarding Credit for Military
Occupations
Documentation appropriate for verifying military occupational
experiences varies according to the branch of service. The links below
are sorted into these different occupations accordingly; each link lists
acceptable forms of documentation for that particular occupation.
Army MOS Verification: Several records may be used for Army
MOS verification. The applicant may submit one or several of the
following forms of documentation:
- Transcript from the Army/American Council on Education Registry
Transcript System (AARTS)
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service"
- DD Form 214, “Armed Forces of the United States Report of
Transfer or Discharge”
- DA Form 2166-5, “Enlisted Evaluation Report (EER)"
- DA Form 2166-5A, “Senior Enlisted Evaluation Report
(SEER)"
- DA Form 2166-1, “NCO Evaluation Report (NCOER)"
- USAEREC Form 10A and EREC Form 10, “Enlisted Evaluation Data
Report"
- DA Form 2-1, “Personnel Qualifications Record"
- DA Form 20, “Enlisted Qualification Record"
- "Individual Soldier’s Report (ISR)"
Army Warrant Officer MOS
Verification: Several records may be used for Army Warrant Officer
MOS verification. The applicant may submit one or several of the
following forms of documentation:
- DA Form 67-8, “Officer Evaluation Report (OER)”
- DA Form 67-8-1, “Officer Evaluation Support Form"
- DA Form 2-1, “Personnel Qualifications Record"
- DA Form 66, “Officer Qualification Record"
- DA Form 4037, “Officer Record Brief”
Navy
Occupation Verification: Several records may be used for Navy
occupation verification. The applicant may submit one or several of the
following forms of documentation:
- NAVPERS 1070/604, “Navy Occupation/Training Awards
History”
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service”
- DD Form 214, “Armed Forces of the United States Report of
Transfer or Discharge"
Navy Warrant
Officer and Limited Duty Officer Verification: Several records may
be used for Navy Warrant Officer and Limited Duty Officer verification.
The applicant may submit one or several of the following forms of
documentation:
- NAVPERS 601-4, “Navy Occupation/Training and Awards
History"
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service"
- DD Form 214, “Armed Forces of the United States Report of
Transfer or Discharge"
Navy Enlisted
Classification (NEC) Verification: Several records may be used for
NEC verification. The applicant may submit one or several of the
following forms of documentation:
- NAVPERS 1070/604, “Navy Occupation/Training Awards
History”
- NAVPERS 1616/24, “Enlisted Performance Evaluation Report"
- NAVPERS 1070/613, “Administrative Remarks"
- Letter of designation from a commanding officer
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service"
- DD Form 214, “Armed Forces of the United States Report of
Transfer or Discharge"
Coast Guard
Occupation Verification: Several records may be used for Coast Guard
occupation verification. The applicant may submit one or several of the
following forms of documentation:
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service"
- DD Form 214, “Armed Forces of the United States Report of
Transfer or Discharge”
- "Achievement Sheet"
Coast Guard Warrant Officer
Verification: Several records may be used for Coast Guard Warrant
Officer verification. The applicant may submit one or several of the

Marine Corps MOS
Verification: Several records may be used for Marine Corps MOS
verification. The applicant may submit one or several of the following

- Individual Training Standards System (ITSS) Maintenance Training
Management and Evaluation Program (MATMEP) Summary Sheet
- DD Form 295, “Application for the Evaluation of Learning
Experiences During Military Service"

Transfer or Discharge"
